1. Required Registration Forms
WHS Grade Release Form - Required for our club to administer the "no-pass/no-play" policy
WHS 0910 Physical Form - Must be signed by your family physician before your player can step on the practice field
Westlake Lacrosse Liability Waiver, Release and Consent Form - hard-copy of release signed electronically during on-line registration. This copy is kept by the coaching staff in case emergency medical care is required.

UIL Academic Eligibility - Document describing the UIL "no-pass/no-play" policy
EISD Extracurricular Athletic Code of Conduct - Eanes ISD's extracurricular code of conduct, including consequences of violating alcohol/drug rules, hazing/bullying, etc.
Westlake Lacrosse Rules of Engagement - Additional expectations of our club, including practice attendance, playing time, attitude, team play, and good sportsmanship.

2010 Sponsor Letter - Sponsorship cover letter describing the club and overview of sponsorship opportunities
